Form Instructions
1. To verify, please check (√) the
“Yes” box, sign and date this order.
2 & 3. Billing and Shipping Information. Please print
or type all information requested.
4. Item Selections. (A) select the desired blend (i.e.,
all 6 bottles of white or red OR a mix of 3 bottles of each in a 6 pak; carbonated or un-carbonated) and container
size (i.e., 500 ml or 1.0 L bottle), (B) indicate the quantity of each selected
item, (C) compute the total cost for each line item, (D) compute sales total
and add all applicable delivery and other charges (Items 6 – 9) and,
(E) calculate order total (Item 10).
5. Payment Method. If you select Item 5a,Credit Card Payment,
please provide your account number and expiration date. If payment by check
or money order (Item 5b) is chosen, please make it payable to “ABI
Ltd”. Sorry, no currency, C.O.D.s or third party checks. Payments
in U.S. Dollars only. All Canadian and International customers, please select
Item 5b and contact our Customer Service Department for delivery charges.
6. Shipping & Handling. All orders are filled as promptly
as possible. Unless otherwise requested and accepted, delivery in continental
U.S. is via UPS BLUE (2nd Day) within 3-4 business days from our receipt
of your order; Alaska, Hawaii, Puerto Rico and Guam, within 4-6 business
days via U.S. Postal Service Priority. Customers are notified of any delay
beyond 10 business days after receipt of their order. Items may be shipped
in multiple boxes, which may arrive separately. All prices are in U.S. Dollars
and F.O.B. Santa Rosa, California, U.S.A. Shipping and handling charges
will be prepaid and added. Title, right to possession, and risk of loss
pass at point of shipment. While we make every effort to ensure the accuracy
of all information, we reserve the right to make corrections if an error
does occur. Please check all addresses carefully. Postal and delivery services
will not forward. We guarantee delivery only to the shipping address we
are given.

Canadian & International Orders. We welcome our Canadian
and international customers. Normally, international shipments are within
10-15 business days from our receipt of your order. Please note that you
will be responsible for all duties, tariffs, taxes or other restrictions.
Additional shipping charges may also apply so please call, fax or write
to our Customer Service Department. All Canadian and international orders
must be prepaid by credit card or check in U.S. funds drawn on a U.S.
bank or Postal Money Order in U.S. funds.
Returns Policy. No matter what the reason, we will gladly
exchange, credit or refund, excluding delivery charges, if you are not
completely satisfied with the product. No questions asked! However, we
would appreciate an explanation in order to better understand customers
and assist in the improvement of our product. Please call our
Customer Service Department within 10 days of receipt of your order for
Return Authorization Number (RA#). Shipments older than 15 days
are not returnable for exchange, credit or refund. Carefully repack the
items, label the outside of the box with the RA# and return (freight prepaid)
to us. Be sure the package is also insured. Return items that do not have
RA# clearly marked on the outside of the box and C.O.D. returns cannot
be accepted.

to 4:00 PM (PT) Saturday, or leave us a voice-mail after hours, on weekend
